We will acknowledge receipt of your information and will usually be able to completely respond to your proposal request within 72 hours.  To expedite this request, please supply:

• Copy of prior year audit report.
• Copy of prior year 5500 - DO NOT include the schedule SSA as this has individual participant information including social security numbers.
• Copy of the most recent Summary Plan Description (SPD) - it is also acceptable to submit the official Plan Document along with all major amendments.
